Athletic Physicals

Bellevue West and Bellevue Public Schools sports physicals should be completed after May 1 of the upcoming year to ensure that the physical lasts the entire school year and through all three sports seasons. Please pick up the proper sports physical form in the office at Bellevue West or download the attached version and return it to the Bellevue West Athletic Office as soon as possible.
